Semisynthetic opioid agonist with relative selectivity for the mu-opioid receptor. Vicodin acts as an agonist that binds to and activates opioid receptors in the brain and spinal cord, which are coupled to G protein complexes and modulate synaptic transmission by adenylate cyclase.

Neonatal Opioid Withdrawal Syndrome, prolonged use during pregnancy can result in withdrawal signs in the neonate. Concomitant with alcohol, CNS depressants (eg, sedatives, anxiolytics, hypnotics, neuroleptics, other opioids), risk of hypotension, deep sedation, coma, respiratory depression, and death. Difficulty swallowing, underlying gastrointestinal disorders resulting in a small gastrointestinal lumen. Reduced intestinal motility, opioids decrease peristaltic waves of propulsion in the gastrointestinal tract and reduce intestinal motility. 

Monitor patients with biliary tract disease, including acute pancreatitis. Risk of prolongation of the QTc interval monitor in case of CHF, electrolyte disturbances, bradyarrhythmias, or concomitant with substances that prolong the QTc interval. Children, safety, and efficacy have not been established. More susceptible elderly, dependency risk.

Liver Failure

Caution in severe IH. Reduce the dose and monitor adverse events, such as respiratory depression, sedation, and hypotension.

Renal Failure

Vicodin Interactions

Plasma concentrations increased by: CYP3A4 inhibitors such as macrolide antibiotics (eg, erythromycin), azole antifungal agents (eg, ketoconazole), and protease inhibitors (eg, ritonavir).

Plasma concentrations decreased by: CYP3A4 inducers (rifampin, carbamazepine, and phenytoin), strong laxatives (eg, lactulose)

Increased risk of respiratory depression with: other CNS depressants, including sedatives, hypnotics, tranquilizers, muscle relaxants, general anesthetics, phenothiazines, other opioids, and alcohol.

Reduced analgesic effect and precipitate withdrawal symptoms with mixed agonist/antagonist analgesics (pentazocine, nalbuphine, and butorphanol) and partial agonist analgesics (buprenorphine).

Hydrocodone Pregnancy

There are no adequate and well-controlled studies of the use of hydrocodone during pregnancy. Long-term use of opioid pain relievers during pregnancy can cause neonatal opioid withdrawal syndrome. In animal reproduction studies with hydrocodone in rats and rabbits, no embryotoxicity or teratogenicity was observed.
